problem with my LVM and fstab

folks, I have a problem when I set up my LVM in fstab I get a constant scroll of errors at reboot. I believe I have setup my LVM correctly I must be doing something wrong in the fstab. I'll post everything I can think of that might be useful to help me fix it.

system: Distro.......: Ubuntu 18.04.3 LTS Kernel.......: Linux 4.15.0-65-generic

contents of /etc/fstab

Reading volume groups from cache. Found volume group "plex_media" using metadata type lvm2

sudo update-initramfs -u was done

and the errors i am getting is
